Four Seasons provides transitional care in partnership with Hartlepool

A partnership between Hartlepool Council and Four Seasons Health Care has enabled a dedicated unit to be provided for elderly people normally living at home who require rehabilitation or transitional care.

Twenty new beds provide rehabilitation and transitional care at the town’s West View Lodge care home. Chosen out of eight homes in the region, West View Lodge put forward a submission to work with Hartlepool Council on this new approach and was awarded the contract.
