Primecare to provide link nursing

Primecare, part of the medical services division of Nestor Healthcare Group, has signed a contract with Welwyn and Hatfield Primary Care Trust (PCT) to provide link nursing support to three nursing homes in the area. To be administered by Primecare Managed Healthcare, the service will provide additional support to help reduce unplanned hospital admissions of nursing home residents and alleviate the likely pressure they would otherwise place on hospital resources. The Primecare Link Nurse team works with the PCT, nursing home staff and associated GPs to help identify and solve care issues within the nursing homes. Services provided include educating nursing home staff through seminars and liaising with other community and acute care resources to help meet the needs of residents. The contract builds upon the out-of-hours GP deputising service already delivered to the PCT by Primecare Primary Care.
